Supporting Your Needs No Matter Your Health Journey

The Adams County Health Department is one of the only health departments in the state to offer diabetes education, prevention, and heart health support to community members! Each of our programs is designed to provide support and guidance, no matter where you are in your health journey.

Journey to Wellness: A Diabetes Prevention Program

This year-long program will teach you the skills to live a healthier life. Learn to lose weight, eat healthy, get active, overcome barriers, and find the motivation to change.

Living with Diabetes

This series of diabetes group classes, also known as Diabetes Self-Management Education and Support, is open to people with all types of diabetes and their support networks. Learn to monitor and reach your blood sugar goal, spend less on health care, and reduce diabetes symptoms.

Heart Health Screenings and Support

No-cost heart health screenings and one-on-one coaching are available to help you identify and reach your health goals. Screenings include cholesterol, triglycerides, blood sugar, blood pressure, and more. More information coming soon!
