HCP: A Program for Children and Youth with Special Health Care Needs

Our HCP team is here for you, providing information, resources, and valuable community support at no cost. No diagnosis is needed, and there are no income restrictions. Please feel free to call or email to find out how we can help.

How Can We Support You?

HCP connects children and youth with the care they need. Through working one on one with our HCP Care Coordinators, families gain a deeper understanding of their children’s condition and are equipped with skills and tools to support their children’s health care needs.

Coordinators can connect you with

  • Developmental evaluations
  • Primary and specialty care
  • Community services
  • Financial resources and insurance assistance
  • Family support groups
  • Transition services for youth moving into adulthood
  • And answers to other questions affecting your family

Together, we identify and prioritize your child’s needs, support you in advocacy, and create a plan to help you reach your goals.
