Every Vaccine Counts

An immunization record is a history of all your or your child’s vaccines. Vaccines you got in another state or country count and are valid, as long as you have paperwork from an immunization information system, a healthcare provider, or a clinic. It is important to keep an accurate, up-to-date record of all the vaccines you have received.

Places to Check for Your Record

  • Check with your current or previous health care provider or public health clinic. Remember that vaccination records may only be maintained for a few years.
  • Ask your parents or caregivers if they have your childhood records.
  • Your home or your parents’ home. Look through old papers, baby books, and school or camp forms.
  • Check with your school, university, or college health service. Like medical providers, schools may only keep records for a short time after a student leaves.
  • Ask previous employers if they may have required immunizations.

Colorado Immunization Information System (CIIS)

The Adams County Health Department Immunization Clinic participates in CIIS, Colorado’s opt-out immunization registry. CIIS is a confidential, computerized system that collects immunization data for all people who have received immunizations in the state. CIIS helps health care providers, school child care centers, universities, and people keep track of the vaccines they or their kids have received.

Frequently Asked Questions about CIIS

Also known as Colorado’s immunization registry, the Colorado Immunization Information System (CIIS) is a confidential, population-based, computerized system that collects and consolidates vaccination data for Coloradans of all ages from a variety of sources. CIIS is a program within the Immunization Branch of the Colorado Department of Public Health and Environment. CIIS helps health care providers, schools, child care centers, and individuals keep track of the shots they and/or their children have received.

The schedule of shots recommended for you/your child to stay healthy and required for you/your child to attend school, child care, camp, and university can seem complicated. Keeping all shots in one place, and on one record, helps ensure you/your child get the complete schedule of recommended immunizations – no more and no less.

If your/your child’s health care provider participates in CIIS, they can enter your/your child’s vaccine records in the system. Other information, including parent/guardian names (for individuals under 19 years of age), gender, Vaccines For Children eligibility status (for individuals under 19 years of age), address, contact phone number(s), email address, date of birth and the office where each shot is given can be included in CIIS to ensure your/your child’s record is accurate and cannot be confused with another patient’s record. CIIS has signed agreements with all participating sites that are authorized to provide information to or access information from the immunization registry. CIIS and all people and entities that access immunization records are required to maintain the confidentiality of those records.

In addition to helping you keep track of your/your child’s shot record, CIIS also:

  • Consolidates information for any shots you/your child has received from multiple healthcare providers into ONE record.
  • Allows your health care provider to see what shots are recommended to keep you/your child healthy.
  • Helps ensure you/your child does not miss any shots or get unnecessary shots.
  • Provides an official copy of your/your child’s immunization history to health care providers, schools, and child care centers.
  • Allows for the recording of vaccine exemptions claimed for you/your child.
  • Allows your health care provider to remind you when it is time for your/your child’s next shot.
  • Keeps records safe during natural disasters or other emergencies.
  • Helps prevent disease outbreaks in your community.

Information in CIIS can only be released to:

  • You (through your health care provider or your formal CIIS record request).
  • Your/your child’s health care provider or hospital providing care to you/your child.
  • A school, child care, or university where you/your child is enrolled.
  • A managed care organization or health insurer where you/your child is enrolled.
  • Persons or entities who have an agreement with the State of Colorado for immunizations.
  • The Colorado Department of Health Care Policy and Financing if you/your child are enrolled in Medicaid.

Yes! You may request copies of your/your child’s immunization record by completing the Request to Release Immunization Record Form. Forms must be notarized and submitted to CIIS with a legible photocopy of your driver’s license or other secure and verifiable identity document. Prior to completing the form, please first contact CIIS to confirm the requested record is in the system. Please note CIIS does not contain complete immunization records for ALL Coloradans.

You can choose to have your/your child’s immunization information excluded from CIIS at any time. Please contact your health care provider, our clinic, or explore more information on how to opt out of the system.

Opt Out of CIIS
